Privacy and governance
This page describes the collaboration form as it actually behaves. It is not a completed legal policy, and it does not claim a retention schedule or an access-control program.
The collaboration form
The collaboration form asks for first name, last name, email, organization type, country, and a message. Credentials, organization, professional title, city, a province or state when the country is Canada or the United States, mobile phone with a calling-code country, area of interest, how you would like to collaborate, and a topic are also on the form. The topic is General or PHENO-001. For PHENO-001 the message is labelled “Your suggestion” and is still the same required message field. An optional checkbox reads “Please don’t quote me by name.” Checking it would not hide the sender. A future live form, if a destination is approved, would still identify the person by the name and email they entered. That live form is not built here. A hidden subject line and a honeypot field are present for a future setup. They do not add a question you are meant to answer. Nothing typed into the collaboration form is collected. The country and calling-code lists are files on this site. Choosing one does not contact a location service. After a local check with no errors, the page can show a notice marked Demonstration only. That notice is not a receipt.
Form not yet available for submissions. The collaboration form does not send, store, or forward what you type. No inbox is connected. Because the site creates no message record, there is no retention period, no administrator access, and no correction process to describe. Those practices would have to be written after a destination is approved and a real submission has been tested. They are not written here as if they already apply.
Do not include health information, patient information, images, or identifiers. The collaboration form is not a medical consultation and not a way to join a study.
No form service is connected
The collaboration form does not transmit entries to a form service. No destination is connected. This page does not claim legal compliance.
Research data is not on this website
No hospital research data is integrated here. A future federated study would need investigator leadership, ethics and privacy oversight, and institutional agreements. The collaboration form must not be used as consent or as recruitment.
Names of companies, hospitals, and publications on other pages are context. They are not endorsements or partnerships unless a later page says so. This site does not say so.
